چکیده

The concept of scleral stiffening therapies has emerged as a novel theoretical approach for treating the ocular disorders glaucoma and myopia. Deformation of specific regions of the posterior eye is innately involved in the pathophysiology of these diseases, and thus targeted scleral stiffening could resist these changes and slow or prevent progression of these diseases. Here, we present the first systematic screen and direct comparison of the stiffening effect of small molecule collagen cross-linking agents in the posterior globe, namely using glyceraldehyde, genipin and methylglyoxal (also called pyruvaldehyde). To establish a dose-response relationship, we used inflation testing to simulate the effects of increasing intraocular pressure in freshly harvested rat eyes stiffened with multiple concentrations of each agent. We used digital image correlation to compute the mechanical strain in the tissue as a metric of stiffness, using a novel treatment paradigm for screening relative stiffening by incubating half of each eye in cross-linker and using the opposite half as an internal control. We identified the doses necessary to increase stiffness by approximately 100%, namely 30 mM for glyceraldehyde, 1 mM for genipin and 7 mM for methylglyoxal, and we also identified the range of stiffening it was possible to achieve with such agents. Such findings will inform development of in vivo studies of scleral stiffening to treat glaucoma and myopia.

Initial studies using aliphatic beta-nitro alcohols for therapeutic corneal cross-linking.

PURPOSE Corneal collagen cross-linking through UVA-riboflavin photochemistry (UVAR) has been shown to be an effective treatment for keratoconus and related keratectasias. In recent studies using sclera, the authors observed that short-chain aliphatic beta-nitro alcohols can cross-link collagenous tissue under physiologic conditions. Thus, this study was undertaken to evaluate these agents as po...

Comparative Study of the Effect of Indomethacin and Acetaminophen on Pain after Collagen Cross-Linking Surgery

Introduction: One of the surgeries that needs to be considered after pain and discomfort is corneal cross-linking of the cornea. The aim of this study was to compare the effect of indomethacin and acetaminophen oral surgery before surgery on postoperative pain in the patients undergoing collagen cross-linking surgery. Methods: The present study was a double-blind clinical trial study that was ...

Immobilization of the Alkaline Phosphatase on Collagen Surface via Cross-Linking Method

Background: Collagen, the most abundant protein in the human body, and as an extracellular matrix protein, has an important role in the fiber formation. This feature of the collagen renders  establishment of the structural skeleton in tissues. Regarding specific features associated with the collagen, such as, formation of the porous structure, permeability and hydrophilicity, it can also be use...

Biomechanical effects of intraocular pressure elevation on optic nerve/lamina cribrosa before and after peripapillary scleral collagen cross-linking.

PURPOSE To evaluate the biomechanical effect of intraocular pressure (IOP) elevation on the optic nerve/lamina cribrosa complex (ON/LC) and peripapillary sclera (PS) of porcine eyes before and after localized collagen cross-linking. METHODS Eighteen porcine globes were divided evenly into three groups. The optic nerves were transected to expose the ON/LC, and each globe was infused through an...

A Review of Collagen Cross-Linking in Cornea and Sclera

Riboflavin/UVA cross-linking is a technique introduced in the past decades for the treatment of keratoconus, keratectasia, and infectious keratitis. Its efficacy and safety have been investigated with clinical and laboratory studies since its first clinical application by Wollensak for the treatment of keratoconus. Although its complications are encountered during clinical practice, such as inf...
